GLP-1s Can Help You Lose Weight. But Are You Losing the Right Kind?
The scale may be moving in the right direction, but preserving muscle, bone, and metabolic health matters too.
GLP-1 medications such as Ozempic, Wegovy, Mounjaro, and Zepbound have changed the way we think about weight loss.
They can reduce appetite, improve metabolic health, and help people achieve levels of sustained weight loss that can be difficult through diet and exercise alone.
But there is an important distinction worth understanding:
Losing weight and becoming healthier aren’t always the same thing.
When the number on the scale drops, it doesn’t tell you exactly what you’ve lost. Some of that weight can come from fat, but some can also come from lean tissue, including muscle.
And that’s where the conversation around GLP-1s gets more interesting.
The Lean Mass Problem
One of the biggest concerns during significant weight loss is the loss of lean body mass.
Lean mass includes your muscles, bones, connective tissue, organs, and other non-fat tissue. Muscle is particularly important because it helps you move, supports your joints and skeleton, and plays a major role in how your body uses energy.
The scale can’t tell the difference between losing a pound of fat and losing a pound of muscle.
But your body can.
Research looking at GLP-1 medications and body composition suggests that a meaningful portion of weight lost during treatment can come from lean mass. That’s why successful weight loss shouldn’t be measured by the scale alone.

What Happens to Your Bones?
Muscle isn’t the only concern.
A 2024 randomized controlled trial followed 64 adults taking once-weekly semaglutide for 52 weeks. The researchers observed reductions in bone mineral density at the hip and lumbar spine compared with placebo, along with changes suggesting increased bone breakdown.
That doesn’t mean GLP-1 medications are inherently bad for your bones.
But it does highlight why resistance exercise, adequate nutrition, and maintaining lean mass deserve attention during significant weight loss.
For people already experiencing age-related changes in bone health, this becomes even more important.
The Mitochondria Connection
There’s another piece of the puzzle that gets less attention: your mitochondria.
Mitochondria are the structures inside your cells responsible for producing much of the energy your body uses every day. They help power muscle contractions, brain function, metabolism, and cellular repair.
When calorie intake drops dramatically, the body is operating with less available energy.
Combine that with the loss of muscle, which contains a high density of mitochondria, and it’s reasonable to ask how significant weight loss may affect the body’s overall energy capacity.
The direct relationship between GLP-1 therapy, lean-mass loss, and mitochondrial stress is still being studied. It’s an emerging area of research rather than an established clinical conclusion.
But it reinforces an important point: maintaining muscle during weight loss isn’t just about looking stronger.
It’s about maintaining the metabolic machinery that keeps your body functioning well.
The Blood Sugar Paradox
GLP-1 medications are well known for improving blood sugar regulation.
But muscle plays an important role here, too.
Skeletal muscle is one of the body’s major sites for glucose disposal after a meal. Muscle cells use a transporter called GLUT-4 to move glucose from the bloodstream into the cell, where it can be used or stored.
When you maintain more healthy muscle tissue, you maintain more of the machinery involved in this process.
That creates an interesting consideration during GLP-1-assisted weight loss:
The medication can improve blood sugar regulation while you’re taking it, but preserving muscle may also be important for maintaining your body’s own metabolic capacity.
That’s one reason resistance training and adequate nutrition should remain part of the conversation.
Where Does Creatine Fit?
This is where creatine becomes interesting.
Creatine monohydrate is one of the most extensively studied sports supplements, and its strongest evidence is around supporting muscle performance and lean mass when combined with resistance training.
For someone losing weight, particularly someone whose appetite has been significantly reduced, maintaining muscle can become more challenging.
Creatine doesn’t replace protein or exercise, but it may provide additional support for maintaining and building lean mass when paired with resistance training.
Research has also explored creatine’s potential effects on bone health, glucose metabolism, and cellular energy, although the strength of evidence varies across these areas.
So it’s better to think of creatine as one piece of a larger muscle-preservation strategy, rather than a solution to every potential challenge associated with GLP-1 treatment.
What Should a GLP-1 User Focus On?
If you’re using a GLP-1 medication, the basics still matter.
1. Prioritize protein
The original recommendations suggest aiming for approximately 1.2–1.6 grams of protein per kilogram of body weight per day, although individual needs can vary. Reduced appetite can make this surprisingly difficult, so protein-rich foods or supplementation may sometimes be useful.
2. Keep resistance training in your routine
You don’t need to become a bodybuilder.
Two or three moderate resistance sessions per week can provide a valuable training stimulus. Bodyweight exercises, resistance bands, machines, or light weights can all have a place depending on your fitness level.
3. Consider creatine monohydrate
A commonly studied daily amount is 3–5 grams of creatine monohydrate. Timing is less important than taking it consistently.
4. Look beyond the scale
Your weight is only one measurement.
Strength, muscle mass, energy, physical function, nutrition, and overall health are all worth paying attention to during significant weight loss.
A Better Way to Think About GLP-1 Weight Loss
GLP-1 medications can be powerful tools for people who need them.
But medication is only one part of the picture.
If you’re losing substantial weight, the goal shouldn’t simply be to watch the scale fall. It should be to lose excess fat while preserving as much muscle and physical function as possible.
That’s where protein, resistance training, adequate nutrition, and potentially creatine can fit into the bigger picture.
Creatine isn’t a magic pill, and it isn’t a substitute for medical care or a good training and nutrition plan.
But when used as part of that bigger strategy, it may be a useful tool for supporting the muscle and cellular energy systems that matter during weight loss.
The number on the scale tells you how much weight you’ve lost. Your body composition tells you what you’ve actually lost. And that difference matters.
