In a groundbreaking study, researchers at University College London (UCL) have demonstrated the effectiveness of semaglutide, a weight loss drug, in aiding patients who struggle with weight regain after bariatric surgery. This study, published in Nature Medicine, presents promising results that could transform post-surgery weight management strategies.

Bariatric surgery is often a last-resort treatment for severe obesity, altering the digestive system to aid weight loss. However, not all patients experience the desired outcomes. Approximately 20% of individuals either fail to lose sufficient weight or regain weight after surgery. With nearly half a million surgeries performed annually worldwide, the demand for effective secondary weight management options is substantial.

Semaglutide: A Promising Alternative

The BARI-STEP trial, led by Dr. Janine Makaronidis at the UCL Centre for Obesity Research, focused on individuals who did not benefit adequately from bariatric surgery. Over 68 weeks, 70 patients received either semaglutide or a placebo, alongside lifestyle counseling and a calorie-reduced diet.

The findings were striking: 85% of the semaglutide group lost at least 10% of their body weight, compared to just 7% in the placebo group. Furthermore, 62% managed to shed at least 15%, and 47% lost 20% or more. Most of the weight loss was attributed to reductions in body fat rather than muscle mass. Improvements were also noted in metabolic health, including better blood sugar and cholesterol levels.

Expert Insights and Future Directions

“Sustainable weight loss strategies continue to focus on long-term lifestyle and health improvements,” experts say. The study’s results underscore the potential of combining bariatric surgery with pharmacological interventions like semaglutide for more effective obesity treatment.

While semaglutide’s side effects, such as nausea and reduced appetite, are known, the benefits could outweigh these concerns for many patients. However, larger and more diverse studies are necessary to fully understand its potential and to explore its benefits pre-surgery.
